The Enrollment process is just 4 easy steps:

1. Request an Enrollment packet

2. Complete the paperwork

Please make sure to include:

- Childs legal name
- Requested program time
- How many days a week
- Transportation if needed
- Medicaid number
- Current DX
- CASII score & level

3. Fax the completed paperwork back to 702.434.2485

4. You will receive a call to schedule an assessment interview.
